Preparing for Your First Visit
- New patients should arrive at the office 15 minutes prior to their scheduled appointment
- A parent or legal guardian must accompany the child.
What to Bring
- Parent/guardian ID
- Patient insurance card
- Referral from your child's primary care physician (if required)
- List of current medications and dosage
- Any recent test results related to your child's condition
- Fill out our new patient forms below
